DESCRIPTION:We will be honoring the retirement of our Chief Medical Officer\, Dr. Gwendolyn Graddy\, and the legacy she leaves of providing extraordinary care\, compassion\, and love for our participants\, caregivers\, and staff.\n\nThe Corner Ballpark (Detroit PAL)\n1680 Michigan Ave\, Detroit\, MI 48216\n\nAll proceeds will benefit our Community Giving Fund which provides basic and emergency needs to our seniors as well as enriching and engaging opportunities.\n\nPACE Southeast Michigan is a unique health care plan that provides home and community-based care and services to help seniors maintain their independence. Part of the national PACE – Program of All-Inclusive Care for the Elderly – PACE Southeast Michigan brings together all the care needed in an integrated care model that allows participants to remain independent in their own homes for as long as possible.
